How do I submit a case with my scanner?

To submit a case with your scanner, follow these steps:

**3Shape:** Log in to your 3Shape Communicate account, send a Connection Request to Forever Bright, and we'll appear in your lab list.

**Itero:** Find Forever Bright in your scanner or online iTero Account and send a connection request. Alternatively, call iTero Support at 800-577-8767 to add us. Restart your scanner to see us.

**Carestream:** Complete a scan and export a raw .STL file. Email the files to info@foreverbrightdental.com or attach them through our My Account portal. You can also visit Carestream Connect on your scanner, search and add Forever Bright, and select us from the drop-down when creating cases. If not visible, enter info@foreverbrightdental.com.

**Cerec (Sirona):** Log in to your Cerec Connect account, go to My Cerec Connect, then Edit Account. Under My Account, select My Favorite Laboratories, search for Forever Bright, and click ADD.

**Raw .STL Files:** Complete a scan, export a raw .STL file, and email the files to foreverbrightdental@gmail.com.

All prescriptions must include a doctor's signature. Reference our turnaround time listing for information about delivery dates.
#2 - PACK AND LABEL BOX
Shipping to your office is no problem, simply send your case to us and our team will take it from there. 
#3 - SCHEDULE PICK-UP
Complimentary pick-ups are available same or next day within the Escambia county area. Fill out the below form or call our friendly team to arrange.

USPS Business Reply Mail
 • Inbound: No charge with our prepaid USPS Business Reply Mail shipping label. Prepaid USPS Business Reply Mail inbound shipping labels can be ordered by calling Customer Service at 850-418-7413. Shipments typically arrive at the lab in 2–4 business days. We recommend placing your sealed case box in a Priority Mail bag (available from your post office). Then, affix our prepaid USPS Business Reply Mail shipping label to the outside. Please note, USPS Business Reply Mail does not have a tracking number.
 • Outbound: $15 Shipments will arrive at your office in 2–4 business days. Each shipment has a bar code with a tracking number to track the package en route to your office.
